Demographics
- Collection Date/ Time - Sampling date and time
- Result Submission ID - Unique ID given by the lab to identify each report
- Date Test Requested - Date of initial well test request
- Latitude / Longitude - Decimal degrees NAD83
- Sampling Point - Location water was sampled from
- Well Permit Number - Dutchess County Department of Health assigned well permit number, if available
- Treated Sample - Was water sampled after treatment (vs. raw)?
- Free Chlorine Concentration - Reading of Chlorine test from water sampled in milligrams per liter
- Field Sample ID - Identifier given to the sample(s) in the field
- Date Samples Received - Date sample received by the testing lab
- ELAP # - NYS Department of Health ELAP (Environmental Laboratory Approval Program) number
- Matrix - Sample matrix as specified by the lab
- Report Date - Date testing results were completed
- Data Submitted to Client - Were test results sent to the client?
- Comments - Any additional notes
- Reason for Testing - Purpose or category of private well test testing

Test Results
- Analyte Name - Name of analyte being tested for
- Date/Time of Analysis - Date and time of analysis
- Method Code - EPA method code for the type of test performed
- Result - Numeric value for testing result
- Detect Flag - Was the analyte detected in the sample?
- Failed to Meet Standard - Does the result exceed the given standards?
- Detection Limit (DL) - Detection limit of lab test
- QA Lab Codes - Quality Assurance lab codes
- MCL - New York State Maximum Contaminant Level
- J = Result estimated below quantitation level
- Est = Estimated Value
